Sister Grace provides care for patients with chronic conditions such as asthma, diabetes, emphysema and hypertension.
Sister Grace is both a registered nurse. She can provide the following services:
Practice Pharmacists support us by helping to monitor and review patients with long term health conditions such as Asthma, Diabetes and COPD. They also support us with medication reviews with all patients, including mental health patients.
Clinical pharmacists work as part of the general practice team to improve value and outcomes from medicines and consult with and treat patients directly. This includes providing extra help to manage long-term conditions, advice for those on multiple medicines and better access to health checks. The role is pivotal to improving the quality of care and ensuring patient safety.
